| خلاصه مقاله | Abstract: Current treatment guidelines for the management of asthma recommend inhaled therapy as the primary route for administering β2-agonists, anticholinergics, and corticosteroids. Therapeutic aerosols are produced by nebulizers, pMDIs , and DPI devices. Each of these delivery systems has advantages and limitations that should be considered in selecting a device for use by an adult or a pediatric patient. Poor inhaler technique leads to: • poor asthma control • increased risk of exacerbations and • increased adverse effects. • Most patients (up to 70–80%) are unable to use their inhaler correctly. A dry powder inhaler (DPI) is a breath-actuated device containing micronized drug particles with a mass median aerodynamic diameter (MMAD) of less than 5 micrometers that are usually aggregated with carrier particles (such as lactose or glucose) of greater diameter. Drug is delivered to the airways by the inhalation of air over a punctured drug-containing capsule or blister. Hold the inhaler straight up with the pink/red portion (the base) on the bottom. Remove the cap while it is in the upright position to make sure you get the right amount of medicine with each dose (Twist the base anticlockwise and then back in the other direction until you hear a click. Your Turbuhaler is now loaded with 1 dose of medicine.) Breathe out normally - do not exhale into the device. Place the mouthpiece into your mouth, with the mouthpiece facing towards you, and close your lips tightly around it. Inhale dose with a rapid and steady flow Remove the mouthpiece from your mouth and hold your breath for 5 to 10 seconds (or as long as you comfortably can). When you exhale, be sure that you are not exhaling into the device. Immediately replace the cap and turn in a clockwise direction as you gently press down until you hear a click. |
