| خلاصه مقاله | Parenteral nutrition (PN) was first used in premature infants 50 years ago, and now this method is used as a standard care in neonatal intensive care units. It is a life-saving tool for babies who cannot tolerate feeding. The purpose of PN is to establish a growth rate that is almost equivalent to the intrauterine growth of a normal fetus with the same post-conceptional age. Also, maintaining the balance of fluids and electrolytes and delivering micronutrients and vitamins are among the goals of intravenous nutrition. To provide a standard PN, all Composition, Prescription, Preparation, Administration and Monitoring steps must be properly planned and executed. In our country, despite the efforts that have been made, standard PN faces challenges in almost all of the above stages.
One of the challenging cases is the preparation of solutions and consumable items for PN. Especially since they are imported and sometimes there is a problem of lack of access to them. However, it should be noted that special 'pediatric' amino acid and lipid solutions should be used in neonatal PN, and the use of adult formulations can be associated with nutrient deficiency and/or complications. Prescribing and preparation of the solutions can be done individually or standardized. In the other hand mixing and preparation of these intravenous solutions should be done with aseptic methods and in a standard clean room to avoid any infectious complications. In order to avoid calculation errors, it is recommended to use an automatic prescribing and mixing system. Administration is done through the peripherally inserted central venous catheter (PICC), and this route is preferable to the peripheral intravenous route.
About 6 years ago, Tabriz University of Medical Sciences has established the country's first standard parenteral nutrition center for newborns with an automatic prescribing and mixing system. . Since then, despite all the challenges and shortages, around 35,000 intravenous serum solutions have been produced in this center and provided to the needy infants. It is hoped that with the support of the country's health authorities, this essential service will be provided to needy babies all over the country. |
