The increase in the aging population and the complications and high cost of hospitalization in institutionalization and the desire of the older adults to live at home for the rest of their lives, home care can be a suitable option. Therefore, every society needs to design and redesign the elderly care system. The present study was developed and designed in order to design a comprehensive model of home-based formal long-term care for the Iranian elderly, and its feasibility was investigated.
Methods
The current study is a multi-method study (explanatory mix method, scoping review, focus group discussion and Delphi). Characteristics of caregivers and care (the type of services provided in terms of basic and instrumental activities of daily living and health services) were conducted through a quantitative study. Then supplementary semi-structured interviews with stakeholders was performed with the goal of achieving the challenges, and solutions for organized home care. In the second step, a scoping review was conducted to identify the way home care is organized in countries. In the third step, the initial model was developed using the results of previous steps. Finally, developed model was validated using the Delphi method.
Results
In the first step, we found that 51.3% of participants had family caregivers and obtained three major themes (service delivery system, service provider and recipient. According to the review study, municipalities act independently in determining the principles and financing of services. Older people are eligible to receive cash and non-cash benefits. Financing is done using insurance, taxes or privately. In the third step, after the initial model was designed. This model can be used as a valid model for delivering home care with CVI 80%.
Conclusion
The present study, by adopting a multi-method approach and taking into account the perspectives of the involved stakeholders, has developed a comprehensive model that can be a guide for policy makers to design integrated home care for older adults using skilled and multidisciplinary caregivers. This model can lead to the quality of care and satisfaction of all stakeholders. |
