Professor of urology and head of organ transplantation ward and head of urology department ,Tabriz university of medical science Background and Aim : The inguinal hernia is one of the most common surgical diseases which sometimes the urologist may be needed for operation because the urologist knows and is familiar with the anatomy of the inguinal canal and in some diseases such as varicocele and understanding testis and radical orchiectomy may be faced with inguinal hernia. Recently mesh was used many times for repairing the inguinal canal which may be associated with some complications for considering these complications instead of mesh we used a flap of the external oblique fascia instead of the mesh and compared it with the result of the mesh. Methods : 50 patients with inguinal hernias were randomly divided into two groups of 25 patients. One group was operated on with mesh and the other group was treated by the flap of the external oblique in the two groups patients were followed for complications in one week and one and 6 and 12 months post-surgery. The results were analyzed by statistical methods. Results : the mean +_SD of age of the group with mesh was 54.04+_10………….. and the mean SD of age group with flap was…49.60+_10.48 …. In the group, the feeling of foreign body in with mesh was…28%… and in the group with the flap was 18%(p=0.002). the sensation of foreign body and pain in the group with a flap was less than with mesh (p<0.05). Hematoma in the first-week post operation there was in 6% of the patients with the flap and in 15%of the patient with the mesh. There was no significant difference between the two groups in terms of disease recurrence rate, infection, and mortality over a period of six months and one year (p<0.05). Conclusion : : the usage of the oblique external flap in the repairmen of the inguinal hernia is associated with less morbidity and complication in comparison with using the mesh and it is cost-benefit.
