Professor of urology and head of organ transplantation ward and head of urology department ,Tabriz university of medical science Background and Aim : A retractile testis is a physiologic phenomenon that according to maturing and increasing the size of the testis will be reduced the severity of the cremaster reflex therefore with puberty it will be in normal condition. the cremaster reflex always is a smooth phenomenon but in rare times it may be associated with severe pain in the testis it needs some intervention for the prevention of damage to the testis as release or resection of the cremaster muscle. Methods : one child with a history of 7 years of retractile testis in both testis with pain in one year with all exams and para clinic evaluation normal another an adult male with isolated retractile testis with associated pain both of them treated by resection of the cremaster muscle through the incision of the inguinal canal Results : In both cases, there was no pain and there was not any complication Conclusion : In the hyper-reflex of the cremaster muscle which is associated with severe pain with no associated other pathologic finding, release and resection of the cremaster muscle is useful.
