Abstract
Introduction: Cervical ectopic pregnancy is an abnormal fetal implantation that can be life-threatening because of the risk of sever hemorrhage. It has an additional importance in association with Placenta accerta. Screening in the first trimester in high risk patients results in early diagnosis and appropriate management.
Case report: A 46-year-old G2P1, with one previous cesarean delivery was referred for mild vaginal bleeding at 10-weeks of gestational age without fetal pole. Pregnancy termination with sub-lingual misoprostol in addition to close maternal monitoring was administered. Patient tachycardia and lower segment uterine hematoma in ultrasound examination after the use of two doses of sub-lingual misoprostol, led us to surgical management due to probable diagnosis of uterine rupture. However, cervical ectopic pregnancy with some degrees of placental penetration to basalis layer of endometrium (acceta type) was the final diagnosis and she finally underwent total abdominal hysterectomy and hypogastric ligation due to uncontrollable life-threatening bleeding.
Conclusion: Cervical ectopic pregnancy which is in differential diagnosis of incomplete or missed abortion, can be accompanied by abnormal placentation. Sever vaginal bleeding can occur and it maybe life threatening. |
