| خلاصه مقاله | Introduction: One of the most popular access methods for thyroid surgery is thyroid resection with an extracervical approach. The bilateral axillo-anterior chest approach (BACA) for endoscopic thyroidectomy is a procedure that enables scar-free neck surgery and access to both thyroid lobes. We successfully present the first attempt at a BACA thyroidectomy in Iran. Case presentation: A 21-year-old female was admitted to our hospital with a neck mass of 1.5 cm that had been present for 6 months. An ultrasound-guided FNAC suggested a papillary thyroid carcinoma in the right lobe. Because the patient desired the best cosmetic outcome, the surgeon decided to proceed with the BACA procedure. The technique was performed to complete a total thyroidectomy on May 22, 2021. The operation time was 170 min, and the patient was discharged the second day after surgery without any major complications. Discussion: This study shows that endoscopic thyroidectomy performed via the anterior chest is a safe and feasible procedure for patients who desire positive cosmetic outcomes with benign nodules of < 5 cm and thyroid malignancies of < 2 cm without extrathyroid extension. The current study is the first clinical experience to use this novel technique for thyroid surgery in Iran. Conclusion: BACA is an acceptable choice for patients who require thyroid surgery and wish to avoid neck incisions. |
