This study was performed to manage IUD insertion as an effective contraception in an appropriate manner. Intrauterine devices (IUDs) are one of the most popular methods of contraception used across the world. In this respect, uterine perforation is one of the most significant complications of IUD use, which commonly occurs at the time of IUD insertion rather than its delayed migration.
Methods:
In this retrospective study, women will be studied in two groups. In the first group, patients underwent surgery to remove an intra-abdominal IUD as well as to correct manage of its complications such as injury to the bladder, small intestine, colon, sigmoid and rectum, as well as patients whom the dislocated device was penetrated to the abdominal wall. In the second group, women underwent surgery only due to remove the lost IUD.
Results:
We found three important key point regarding the uterine perforation as a complication of IUD insertion. As the first point, invisible device strings had been misinterpreted as IUD expulsion without any additional evaluations to localize the lost devices in most of cases. Secondly, most of these women had IUDs inserted postpartum or following abortion and majority of them were simultaneously breastfeeding. This could be a sign of improper timing to place such devices. As the third point, most of the patients had not received ultrasound-imaging evaluations of probable uterine anomalies or uterine structural disorders affecting the risk of perforation prior to IUD insertion, underlining incorrect patient selection for IUD use.
Conclusion:
Proper patient selection is of utmost importance to avoid IUD use complications. Only experienced health care providers can deliver such services. Lost IUDs should be localized and extracted to prevent unexpected complications. Minimally invasive techniques are further advised to retrieve migrated IUDs and laparotomy is only recommended in some complicated cases. |
