| خلاصه مقاله | Urinary incontinence in childhood is involuntary urine leakage after toilet training. Bladder dysfunction can be a presenting sign for child sexual abuse. Therefore, sexual abuse should be considered in the evaluation of children with urinary complaints. In this letter, we present an 11-year-old girl who developed a large vesicovaginal fistula as a result of childhood sexual abuse. She presented with persistent and progressing watery vaginal discharge for three years. Endoscopic examinations of the bladder and vagina showed a foreign body in the vagina and a large fistula between the bladder and vagina. The foreign body was removed and the child underwent fistula repair surgery after complete evaluations. While the girl initially did not report any complaints of childhood violence, after proper emotional support she stated that her caregivers repeatedly used various objects, including small pieces of her own toys, for vaginal and anal penetration after his mother's death. However, when evaluating a young girl suffering from urinary problems, it is important to remember the child sexual abuse as a cause of urinary disorders. So, the physical examination should look for signs of physical or sexual abuse. The identification of child abuse is also required to protect the child from the subsequent and maybe more serious injuries. |
