خلاصه مقالهObjective: Myofascial pain syndrome (MPS) is one of the most common musculoskeletal disorders. Due to its discomforting and disabling nature, choosing an appropriate treatment of MPS is an essential part of musculoskeletal rehabilitation. Dry needling, as a common treatment of MPS, has direct therapeutic effects on the treatment area. However, its remote neurophysiological and mechanical effects are yet to be known. The aim of this study is to investigate the remote effects of upper trapezius trigger point dry needling on pain intensity, sensitivity and muscle stiffness. Method: Forty patients with myofascial pain syndrome with right dominancy and involvement of upper limb were participated in this quasi-experimental study. Dry needling was performed for one session on the right upper trapezius muscle and its effects on both ipsilateral and the contralateral sides were investigated. Pain intensity with visual analog scale, pain pressure threshold with algometer and muscle stiffness at rest and contraction states with sono-elastogram were evaluated before and after the intervention. Results: The findings of current study showed that the pain intensity, sensitivity and muscle stiffness at rest state had a significant improvement both on the treatment and the contralateral sides (p <0.05). However, muscle stiffness did not show a significant change in contraction state on the contralateral side. Conclusion: Improvement of neurophysiological and mechanical properties of the contralateral upper trapezius muscle in patients with MPS indicates the non-local effects of dry needling. Since the trigger point may lead to central sensitivity in addition to peripheral pain, choosing a treatment approach like dry needling that is effective at the central level may be an appropriate treatment for these patients.
